Cardiovascular disease (CVD) is the main cause of death in the Czech Republic. The decline in CVD mortality observed in our country since 1985 may be due to a decrease in its incidence or case fatality. Case fatality is mostly related to the standard of health care whereas the incidence of CVD is associated with the risk profile of the population. The aim of this study was to determine the prevalence of major CVD risk factors in a representative Czech population sample. Methods: A 1% population sample aged 25-64 years was randomly selected from nine districts of the Czech Republic. The examination consisted of completing a standard questionnaire, obtaining...

Background: Currently, the relationship between the severity of chronic heart failure (CHF) and the main quantitative characteristics of the hippocampus, cognitive functions, and lipid profile components in pa- tients with coronary heart disease (CHD) is not clearly understood. Materials and methods. Fifty patients with CHD and functional class (FC) II-IV CHF were examined. The study did not include patients who had severe medical conditions which could develop the morphological changes in the brain. In addition to the routine clinical tests, Doppler echocardiography and nuclear magnetic resonance imaging (MRI) of the brain were performed; the right...

Cor Vasa 2020, 62(1):33-36 | DOI: 10.33678/cor.2019.080

Introduction: Some patients with symptomatic aortic stenosis undergo sternotomy and have the operation aborted because of intraoperative discovery of unexpected severe calcification of the aorta. The aim of this study is to present our experience and analyze the causes, the management, and the outcomes of these patients. Methods: This is a retrospective study. Between January 2010 and December 2017 10565 patients underwent cardiac surgery, 27 patients (0.3%) had aborted sternotomy, of whom seven patients with severe aortic stenosis. The reason was unexpected intraoperative discovery of severe calcified ascending aorta that precluded placing of an aortic...

(Using questionnaires to detect obstructive sleep apnoe)

Pavel Šulc

Cor Vasa 2020, 62(1):44-49 | DOI: 10.33678/cor.2019.086

Introduction: Obstructive sleep apnoe (OSA) syndrome is a serious and rather frequent condition remaining largely undiagnosed and, therefore, untreated. A possible way how to improve the detection of OSA in the general population is the employment of dedicated questionnaires to preselect high-risk individuals. Objectives: The aim of this review is to compare sensitivity and specificity of questionnaires developed to preselect patients with clinically relevant OSA. Methods: Based on eligibility criteria, we selected studies focusing on the sensitivity and specificity of each of the following questionnaires: the Berlin Questionnaire (BQ), Epworth Sleepiness...

(Athlete's heart vs hypertrophic cardiomyopathy - differential diagnostics according to current guidelines)

Martin Bláha

Cor Vasa 2020, 62(1):50-55 | DOI: 10.33678/cor.2019.087

Hypertrophic cardiomyopathy (HCM) is one of the most common causes of sudden death of young athletes under 35 years of age. Its phenotypic manifestation may resemble the physiological response of the myocardium to a high training load in case of so-called athlete's heart. These two modalities must be distinguished from each other in order to preserve the life of athletes with yet unrecognized HCM, and also on the other side, to avoid unnecessary stopping the career of the healthy ones.

(Sanitation of dental foci before valve surgery)

Cor Vasa 2020, 62(1):56-59 | DOI: 10.33678/cor.2019.071

Patients after heart valve surgery or its prosthesis replacement are at an increased risk of developing infectious endocarditis (IE). Routinely performed prevention measures of IE include screening and eventual sanitation of potential sources of infection. Most common site of preventive sanitation procedures is an oral cavity. Even though sanitation of dental foci became a part of routine screening, there is a lack of data to support its benefits to the patients. This article summarizes published clinical trials on this topic. In cooperation with doctors from the Department of Stomatology, 2nd Faculty of Medicine of Charles University in Prague, a...

Cor Vasa 2020, 62(1):72-74 | DOI: 10.33678/cor.2019.045

Patent ductus arteriosus (PDA) is a persistent communication between the descending thoracic aorta and the pulmonary artery that results from failure of normal physiologic closure of the fetal ductus. It is one of the more common congenital heart defects. The PDA closure is the main treatment and can be surgical or percutaneous. The trancatheter approach proved its efficacy and safety all over the past 20 years and a variety of devices have been developed and are now available. This report describes a transcatheter closure of a PDA on an 18-year-old girl with features suggestive of a large PDA. While we intended to close the PDA using an Amplatzer...

Female patient with frequent premature ventricular contraction (PVC) with arrhythmia induced cardiomyopathy with left ventricle systolic dysfunction. At first patient underwent endocardial end epicardial catheter ablation via the coronary sinus. After these procedures a total amount of 36% of PVC activity was verified with 24 hour EKG holter. Therefore after detailed consideration and due to numerous publications we chose retrograde coronary venous ethanol ablation in third branches. The day after the procedure there was significant decrease in total amount of PVC. Control echocardiography upon dismissal showed an increase in systolic function of left...

Cor Vasa 2020, 62(1):80-84 | DOI: 10.33678/cor.2019.011

Permanent unifocal right ventricular pacing can lead to ventricular dyssynchrony with subsequent dysfunction of the left ventricle, which can lead to heart failure. Heart failure caused by right ventricular pacing was traditionally associated with pacing from the apex of the right ventricle. Over the last two decades, significant efforts have been made to find alternative pacing sites that can reduce or prevent the risk of heart failure. Based on the latest findings, pacing the His bundle area appears extremely promising. In this case report, we present a 79-year-old male, who developed left ventricular dysfunction with heart failure within six months...

(Atypical manifestation of giant aneurysm of the ascending aorta)

Cor Vasa 2020, 62(1):85-88 | DOI: 10.33678/cor.2019.053

Ascendant aortic aneurysm has an asymptomatic clinical course and manifests itself with various, often fatal, complications. Due to more accessible imaging methods, the finding of gigantic aneurysm is rare nowadays. We present a case of a patient in whom an acute myocardial infarction revealed the presence of a giant aneurysm of the ascending aorta.

Cor Vasa 2020, 62(1):89-93 | DOI: 10.33678/cor.2019.013

Takotsubo cardiomyopathy (also known as stress cardiomyopathy or apical balloon syndrome) is an acquired myocardial disease characterized by symptoms similar to acute coronary syndrome. Pathophysiological mechanism of this disorder is still not well understood. Generally accepted mechanism is probably catecholamine-induced myocardial stunning provoked by stress, excessive physical exertion or rarely endocrine active tumors. We present a case report of a patient who was admitted to our clinic for suspected acute coronary syndrome. Subsequent examinations showed that it was a stress cardiomyopathy caused by extraadrenally localized tumor - paraganglioma....
