Paravalvular abscess of a mechanical valve prosthesis is a rare but serious complication of infective endocar ditis. It can lead to prosthetic dehiscence, embolization, conduction system disturbances, fistula formation, or mechanical compression of the coronary arteries. This case report describes a 68-year-old man with in fective endocarditis of a mechanical aortic valve prosthesis, complicated by a paravalvular abscess, compression of the left main coronary artery, and the development of acute coronary syndrome. protézy, poruše převodního systému, vzniku píštěle, em bolizaci vegetací nebo mechanické kompresi koronárních tepen. Absces je zánětlivá kolekce kolem chlopenní ná hrady a představuje vzácnou, avšak závažnou komplika ci IE. Paravalvulární absces vyžaduje včasnou diagnostiku a efektivní cílenou léčbu. Popis případu Osmašedesátiletý, plně aktivní muž, s anamnézou náhrady aortální chlopně mechanickou protézou a implantovaným kardiostimulátorem pro atrioventrikulární (AV) blokádu 3. stupně (21 let před přijetím), byl přijat na spádové interní oddělení pro čtyři měsíce trvající recidivující febrilie. Byly vyloučeny běžné infekční zdroje a pro pozitivní nález sta Obr. 1 - Rozsáhlý, prakticky cirkulární absces kořene mechanické aortální náhrady (při TTE vyšetření). TTE - transtorakální echokar diografie. Obr. 2 - Promývané abscesové dutiny aortálního kořene (TTE s ba revným dopplerovským mapováním). TTE - transtorakální echokar diografie.
