Plasma levels of thiocyanate in the Czech population and their genetic determination

Background:Plasma thiocyanate levels are a commonly used marker for smoking status determination. It is not known whether there is any genetic predisposition in terms of plasma thiocyanate levels. We have evaluated possible genetic predetermination of plasma thiocyanate levels in a population-based study. Presence of blood flow through the infarct-related artery prior to direct coronary angioplasty (percutaneous coronary intervention, PCI) is associated with a significantly better course of the disease. Several strategies of drug therapy instituted on first contact with the patient to recanalize an infarct-related artery before PCI have been proposed. Mitral valve repair in patients with left ventricular dysfunction continues to be a subject of debate. A critical consideration when deciding about the procedure is that the patient will benefit in terms of improved quality of life and prognosis.Proper indication requires identification of the etiology of mitral regurgitation and the cause of left ventricular dysfunction. Mitral valve surgery is designed to treat valve disease, myocardial disease, or heart failure in cases of organic mitral regurgitation, ischemic mitral regurgigation, or dilating cardiomyopathy, respectively.

Libman-Sacks endocarditis versus infectious endocarditis

We present a unique case of a 32-year-old female patient with Libman-Sacks mitral valve endocarditis with rapid progres-sion into severe mitral regurgitation and congestive heart failure requiring valve replacement. The need for proper differential diagnosis and optimal timing of surgery is highlighted. The procedure was performed on-pump. The postoperative period was uneventful. Follow-up echocardiography documented proper position of the prosthesis without paravalvular leak.

Patent foramen ovale as a cause of paradoxical embolism in scuba divers

The cause of decompression sickness (DCS) in scuba divers is bubble formation in tissues and venous blood during ascent. Divers with patent foramen ovale (PFO) are at increased risk of paradoxical gas embolism in the brain or other organs. DCS with neurological symptoms may unmask a hitherto asymptomatic intracardiac shunt. The case report of 3 scuba divers investigated for PFO after repeat DCS is presented. In two cases, atrial septal defect and PFO were found and closed successfully with an Amplatz occluder. Exclusion of PFO in the third patient helped to establish the diagnosis of cervical intervertebral disc prolapse.

Development of cardiac surgery over the last decade has made it possible to operate even on patients with serious comorbidities. In addition, the expanding body of experience of cardiac surgery departments has resulted in extended average life expectancy of patients undergoing cardiac surgery. The paper addresses the issue of patients with hematologic malignancies, who had open-heart surgery in our department. Generally, these patients are immunocompromised and, consequently, at increased risk of infections.
